Sacred Heart

The following requirements are to be completed one month BEFORE the baptism date. If we have not received all of the requirements one month before the baptism date, your date may be lost. 

1) You will complete the following at the Baptism Info Night (2nd and 4th Thursday of the month at 7pm):

• Most importantly, please bring your child's birth certificate to the Baptism Info Night. (The birth certificate must be from the county government. Hospital records are accepted upon review.) Originals only, please... no copies! 
• After we copy the birth certificate, you will fill out a Baptism Registration Form with the Requested Baptism Date. Dates must be requested at least six weeks in advance.

2) You can turn in the Baptismal Godparent form for each godparent at any Baptism Class (the first Thursday of the month at 7 pm), at the Parish Center during office hours, or before a Baptism Info Night.  Please click here to read our guide to selecting a godparent.   Godparents can access this form through our For the Godparents page.                               Guide to Godparents  For the Godparents

3) The mother, father and each godparent will need to attend the Baptism Class. Baptism classes are held at Sacred Heart every first Thursday of the month from 7:30-9 pm. For more information on the Baptism Class, please see Baptism Renewal Night. 

If you attend a Baptism Class somewhere other than Sacred Heart, you will need to submit a letter or certificate of completion from the parish where the baptism class is attended, either to the Parish Center or before a Baptism Info Night.
