Baptism Checklist

The following requirements are to be completed one month BEFORE the baptism date. If we have not received all of the requirements one month before the baptism date, your date may be lost.
1) You will need to complete the following at the Baptism Info Night (2nd Thursday of the month at 7pm) to be Registered:- Baptism Registration Form with a legible copy of the County Birth Certificate.
- After turning in the Birth Certificate, you will fill out a Baptism Registration Form with the Requested Baptism Date. Dates must be requested at least six weeks in advance.
2) You can turn in the Baptismal Godparent form for each godparent at any Baptism Class (the first Thursday of the month at 7 pm), at the Parish Center during office hours, or before a Baptism Info Night. 3) The mother, father and each godparent will need to attend the Baptism Class. Baptism classes are held at Sacred Heart every first Thursday of the month from 7:30-9 pm. For more information on the Baptism Class, please see Baptism Renewal Night.
If you attend a Baptism Class somewhere other than Sacred Heart, you will need to submit a letter or certificate of completion from the parish where the baptism class is attended, either to the Parish Center or before a Baptism Info Night.
