Request Certificates/Records

- If you received your sacraments of Baptism, First Communion, Confirmation, or Holy Matrimony at Sacred Heart and are in need of sacramental certificates, please submit the information below. A staff member will contact you within about two business days to confirm your request. You can also stop by the Parish Office, during working hours, and request certificate in person. There is a $10 charge to each certificate requested.
